Usefulness of the Medial Portal during Hip Arthroscopy

چکیده
The current conventional portals for hip arthroscopic surgery are the anterior, anterolateral, and posterolateral portals. For lesions in the medial anteroinferior or posteroinferior portion of the hip, these portals provide insufficient access to the lesion and consequently lead to incomplete treatment. Thus, in such a situation, a medial portal approach might be helpful. However, operators have avoided this procedure because of the risk of injury to the obturator, femoral neurovascular structures, and the medial femoral circumflex artery. Thus, to overcome the disadvantages of the conventional method for medial lesions of the hip, we performed a cadaveric study to evaluate the technique, usefulness, and risk of the medial portal technique.

Medial hip arthroscopy portals: a novel approach for hip pathologies. Is it feasible?
PURPOSE To report 3 patients operated on using medial hip arthroscopic portals, describe the surgical technique and clinical outcomes. METHODS Three medial portals were made, the first one at the posterior edge of the adductor longus muscle (posterior medial portal), the second one at the anterior (anterior medial portal) and the third at the posterior border of the adductor longus, 5 cm dist...
متن کاملSingle-Portal Arthroscopy of the Central Compartment of the Hip.
Since hip arthroscopy has become a standard of orthopaedic practice, the indications have continued to expand as it has proved to be a helpful diagnostic, as well as therapeutic, tool. Access to the hip joint, however, remains challenging for the orthopaedic surgeon who does not routinely perform hip arthroscopy. We present a single-portal arthroscopic technique, showing the feasibility of sing...
متن کاملThe accessory posteromedial portal revisited: utility for arthroscopic rotator cuff repair.
Arthroscopic rotator cuff repair is a technically challenging procedure. Accessory arthroscopic portals have been described that allow for optimal suture anchor placement, suture management, and knot tying. We describe here the usefulness of an accessory posteromedial portal that facilitates direct suture retrieval through the posterior aspect of a rotator cuff tear. This portal is created appr...
متن کاملThe posterior-medial portal.
Repair of the posterior superior labrum usually involves placing an anchor posterior to the biceps attachment through an anterior-superior or Wilmington portal. Access to the posterior-superior labrum can be accomplished by establishing a portal medial to the posterior viewing portal. The initial posterior arthroscopic viewing portal is established slightly lateral to a traditional position to ...
متن کاملTopographic anatomical study of the sciatic nerve relationship to the posterior portal in hip arthroscopy.
OBJECTIVE To evaluate the anatomic topographic relation between the sciatic nerve in relation to the piriform muscle and the posterior portal for the establishment of hip arthroscopy. METHODS We dissected 40 hips of 20 corpses of adult Brazilians, 17 male and three female, six black, six brown and eight white. We studied the anatomical relationship between the sciatic nerve and the piriform m...
